Independent Contractor Forms Group

NOTE: If you need to pay an independent contractor, he/she must submit a group of forms consisting of the following:

• Independent Contractor Questionnaire
  • Adobe PDF
  • Word
• The Service Provider must complete and submit this form along with
  • IRS Form W-9, Request for Taxpayer Identification Number.
• Invoice
• Direct Payment Voucher Form (DPV 88R) (help facility for filling out form)
