SUBJECT: Name and Address Corrections for W2 Purposes
DATE: November 27, 2007

As we all know 2007 is coming to a close, which means it is W-2 time.  Please post the enclosed notice on your bulletin board or distribute it to your employees.  It is very important that we have an accurate Name and mailing address for each employee who is scheduled to receive a W-2 Wage and Tax Statement for calendar year 2007.  If the address is incorrect please submit a PAF to Human Resources for staff employees or the Provost’s Office for academic non-faculty and faculty employees.  If the employee’s name is incorrect please attach a copy of their Social Security Card with a PAF and forward it to the appropriate processing area.
