Colleagues: The University of Chicago will hold free flu shot clinics at several University locations for active, benefits-eligible faculty, staff, and other academic position (OAP) employees, beginning Wednesday, November 1, 2006. These are walk-in clinics - no appointment is necessary. However, appointments can be scheduled with Occupational Medicine by calling 702-6757 (press #1). Please see the program details below. Who is eligible: Free flu shots are available to active, benefits-eligible University of Chicago employees only due to a limited vaccine supply. The program will not be extended to retirees, family members, students, temporary employees, and HR Services Temporary Pool employees. People who are not eligible for this employee benefit may obtain a flu shot for a fee through their regular health care provider, or through neighborhood pharmacies such as Walgreen's and Osco. Students should contact the Student Care Center (SCC) at 702 - 4156 or visit the SCC Web site to learn about the free flu shot program for students. The SCC is located at 5839 S. Maryland Avenue, First Floor, Room 100. What to bring: Eligible University employees should bring their University I.D. Card and a completed Influenza Vaccine Record 2006-2007 to a vaccination site. To obtain the form, visit the link on this page. You must present your University I.D. Card to receive a free flu shot. Walk-in vaccination clinics: Eligible University employees may attend one of the walk-in clinics available at several University locations. Addresses are also listed for your convenience.
