SUBJECT: Social Security Number and Address Corrections
DATE: November 13, 2005

As we all know 2005 is coming to a close, which means it is W-2 time. Please post the following notice on your bulletin board or distribute it to your employees. It is very important that we have an accurate Social Security number and mailing address for each employee who is scheduled to receive a W-2 Wage and Tax Statement for calendar year 2005. If the address is incorrect please submit a PAF to Human Resources for staff employees or the Provost's Office for academic non-faculty and faculty employees. If the Social Security number is incorrect please forward a copy of the employees Social Security Card to James Hamaguchi of the Payroll Department.
